 Vital Information Profile
(VIP)In the future, PHS TM will be able to offer you a VIP service to be used in the event of an emergency. During this emergency, your PHS online service would allow health care professionals to access your VIP profile page. You will create a personal "Vital Information Profile" page regarding specific health care information that you feel would be important in the event you can not answer for yourself. An example of this would be allergies, next of kin or any other pertinent information.
